“Hello,” says Hermione, glancing up from her paperwork. “Draco. Can I…help you?”
He’s well put together, dropping gracefully into the chair across from her desk without invitation. “Yes. I’m in need of medical attention.”
“I see.” Hermione does not, in fact, see. This is quite unorthodox. There’s a triage procedure and rarely does it result in patients coming to her office. “Tell me more.”

It means that there's an underlying fear that Shane will choose someone else over Ilya. And aside from the occasional depressive spiral that can lead Ilya to those thoughts — though that rarely happens after so many years of marriage — he knows that Shane would choose him over everyone. He trusts that Shane loves him just as much as he loves Shane.
Ilya knows there's no chance in hell that a random man flirting with Shane in a pet store would be enough to turn his head.
