Currently, he was in total bliss and at the same time some of the worst pain in his life. That is certainly saying something considering his profession. You can’t be a Pro Hero and bawl when you stub your toe. To be fair, giving birth without your mate in a jail cell was a lot worse than that, but it proves his point regardless.
When a heavily pregnant Katsuki is captured, he's forced to give birth alone in his cell. By the time Izuku finally rescues his mate, it's too late: their pup has been taken.
