Summary
Izuku was supposed to have huge omega titties once he hit puberty, and yet he can barely fill out an A cup once he gets to college. It shouldn't matter because body positivity and omega rights, but it does. It matters a lot when his campus crush is his college's star football player, an alpha named Bakugou Katsuki, whose type is omegas with huge breasts that he can titty fuck to his heart's content.
Unfortunately for Izuku, whose mosquito bites were the bane of his existence all throughout his teenage years, he doesn't stand a chance to gain the alpha's attention. So, he decides to pierce his nipples for the confident boost.
Fortunately for Katsuki, he doesn’t discriminate when it comes to breasts, despite what everyone says. In fact, he might be just a tad obsessed with a pretty omega who happened to have small tits and the metal bars threaded through his nipples.

Registry lists combed through again and again, first with confusion, then with urgency, then with something very close to panic.

And Izuku had gone rabid, he had downright lost his mind, because he was totally okay with being called names and with being belittled; he had dealt with that sort of thing throughout his entire life. He was okay with that, as long as that gruesome villain kept Kacchan’s name out of his filthy mouth.
Izuku had always been like this, even during childhood. Badmouth Kacchan in front of him? Meet his rage, and quite possibly his fists. Maybe that’s just what love is for him. Standing up for someone.
But Kacchan doesn’t love him. Izuku knows that much now. If he did, he wouldn’t have used Izuku to stand up for himself.
Alternatively: Katsuki is in the closet, but he finds out a homophobe is in his pro-hero fanbase. Because of that, he kisses Izuku in front of the man just to make a statement, not aware that Izuku has feelings for him. A lot of trouble ensues.
