Most people think that silence is far preferable to screaming, but Dr Robinavitch knows that the opposite is true in the ER. Screaming means the patient has the ability to maintain their airway, whether that's a construction worker with a crushed pelvis or a newborn baby testing out her lungs for the first time. Silence is a worrying sign.

Summary
Every Madrigal on their fifth birthday gained a soul mark. Felix and Agustin both received one on the day they married their partner. Alma received one the day she lost her husband, the bond between the married couple fading and replacing with a new one. Alma called it a connection to their miracle, to their family.
After all, why else would it be a butterfly?

Until then she'll keep them hidden, even as they weep and spread across her whole body. Even as the darkness threatens to swallow her up.

A German Shepherd-husky mix, ex-military dog called Hunter is paired with Oliver as his psychiatric service dog, and from there on, the archer’s recovery from his five years of trauma, struggle to adapt to the new social commitments of owning a service dog, and his mission to save Starling City, begin.

Oliver came back with more mental health difficulties than he wanted to believe, or let on. However, there's only so much you can hide when you have severe PTSD, Social Anxiety, and Depression. Not to mention the difficulties with sleeping and eating.
The same day he gets surprise introduced to John Diggle, the same half hour even, he also gets introduced to Hunter, his new psychotherapy Service Dog. -
