“You…what?” Dean asks, the beer frozen half-way to his mouth.
Sam flushes ice-cold down to his toes, the exact feeling of hearing a gun cock behind you, the exact feeling of catching a glimpse of something dark and large lunging at you head-on.
He stops dead.
“Nothing.” He says, mouth numb, face numb, fingers numb. His own bottle tilts precariously, and Sam jerks, trying to keep it from spilling over his jeans. He lunges to his feet, trying to play it off, but the adrenaline pulsing through his bloodstream makes it impossible.
Or: Sam accidentally confesses his love to Dean after a hard hunt. Dean doesn't reciprocate. Yet.

He wasn’t even entirely sure why he was there.

The two attendings have a very well deserved day off. This does not mean that Dennis gets the same. So he's at work.
But it's okay. He's paired with Samira, which is fine. He likes her company. So much so that he decides to join her for a bit of air in the ambulance bay before anything goes downhill, as it often does.
Which leads them to this moment, and Dennis isn't sure the outcome is certain.
Or; Samira and Dennis are kidnapped from PTMC, accidentally being entered into a deal they never signed up for.

It's not that he enjoys holding on for hours and hours. He can, yeah, he's cultivated the skill. You don't get through a single fucking shift in a hospital without having to learn to hold your piss.
