“Thank you, Sam.” Cas smiles at the man in front of him, trying hard to offer comfort even though Cas hadn’t been able to explain that he was having a problem. “You’re a good man. I love you, too.”
Sam blinks, for some reason surprised by this response. Cas thinks about how Dean would react in this moment, but Cas doesn’t think making fun of Sam’s hair or calling him a hippie would quite communicate what he intends. Cas hasn’t mastered Dean’s ability to make insults affectionate, and he doesn’t really think this is the moment to try.
“Uhm. Thank you?” Sam says, his tone questioning. Cas plods on anyway. Sam has been the object of Dean’s love for as long as he has existed. A different love, yes, but still, Sam is perhaps the one person who can help Cas find a way to respond.

Summary
The first mixtape Dean received from Cas forced him to confront the fact that Cas does, in fact, love him. The second might force him to entertain the wild possibility that he is also (maybe, perhaps) allowed to love things, too.
Or, Dean builds a dock, attends a concert, and realizes that he's allowed to like things sometimes.

"Dean looks down at the mixtape, looks at Cas’s spindly handwriting: “For You” this time, instead of “For Dean,” like it’s so obviously Dean’s that it doesn’t need his name on it. He decides to let himself have this for just a little while more.
He aims the impala towards South Carolina. He’ll let himself have this selfishness until he gets back from Jody’s. It’s not like he can talk to Cas about it right now anyway.
Dean slides the tape into the deck, lets his fingers linger on the buttons, and smiles to himself. At least this time he knows what to expect.
Dean takes Baby to the open road and lets the tape play."

In other words, Cas returns from the Empty and gives Dean a mixtape. Dean slowly loses his mind.
