Katsuki stares at him, taking in the sight of Izuku in his pajamas and shivering like crazy with bloodshot eyes and disheveled hair. He frowns.
He’s too smart not to immediately realize Izuku isn’t coming from his bedroom, and slowly, silently, takes in the horrible looks of him from head to toe. He’s not okay – but he didn’t come for me this time?
Dark bags under his eyes, pale face, shuddering body. A wave of deep worry hits Katsuki like a truck, and he leaves the damn breakfast behind to approach and reach for Izuku, meaning to pull him into a warm embrace like he’s often taken to doing – he looks like he needs one.
But Izuku immediately flinches away.
Alternatively: Izuku has bad nightmares about the War and doesn't know how to cope. He relies on Katsuki for comfort for a while, until one nightmare about his death makes him decide to put his distance between the two of them. As it turns out, Katsuki doesn't know how to cope with that either, and trouble only increases when Izuku accidentally hurts Katsuki during a UA training.

Izuku groans, as he walks down the hallways during break time, heading towards the teacher's coffee room. He can't blame anyone else but himself for that one. He's been hearing his own voice for the whole day, saying that same sentence over and over, coming from the student's phones.
A video of him scolding a pro hero has become viral; it was during an official event to which Izuku had attended as a fan. A lot of pro heroes went to greet civilians, give a couple of autographs, talk about themselves and what they do for a living, answer to reporters' questions and pose for pics.

At some point Izuku had stopped talking much to anyone from Class A. Sometime in his college days, as he was forced to watch his friends from the sidelines, as they rose to glory and heroism without him– that seemed to be around the time that Izuku’s texts became less frequent, and calls even less so. As he was constantly confronted, day in and day out, with the grief of losing his only dream, the grief of the death of his childhood... he had decided to try his best to just… stay out of everyone’s way. Sink further and further into himself, until he blinked and suddenly, he was 23 – dressed up as a teacher, readying himself for his first day at U.A. – and wholly unrecognizable to himself.

Izuku Midoriya becomes a vigilante after the war, still grasping to his dream of being a hero. He doesn't know about the suit, and Katsuki doesn't know about Izuku's life outside of teaching (yet).

He must’ve been dreaming. Or hallucinating. Or having some strange, out-of-body experience.
The word fell from Katsuki’s lips nonchalantly, like it was the most normal thing to bring up in casual conversation. Izuku blushed furiously. He felt the burn in his cheeks and ears; his entire face had to be bright red. The word kept repeating in Izuku’s mind, the echo of it bouncing off the walls of his skull in his best friend’s alluring baritone.

or workaholic izuku struggles to relax while balancing school life, hero life, and personal life. obviously, his only solution is a friends-with-benefits relationship with his best friend, which definitely won’t affect their secret undercover mission at all.
