On the ancient, rusted-out wreck of the small spacecraft below the dune, several desiccated corpses had been strung up by their ankles—probably by the Bad Lads or some other local gang—and were being picked at by an assortment of Worms and buzzards. Next to them, however, were two men who were still very much alive, tied up back-to-back and dangling upside down, slowly turning in circles as the pair wriggled and squirmed in their bonds.
They weren’t trying to escape, though. They appeared to be much more focused on trying—and failing—to punch each other’s lights out.
“I’m not buying it,” Meryl muttered.
Roberto scoffed. “They sure act like a pair of brothers. Look like it, too.”

Here’s the thing: Vash Saverem is broke. Not just poor — oh, no. He’s out-on-the-street broke. He’s three eviction notices down and the last one was printed on pink paper. Fired from two jobs within a week flavor of broke. He hasn’t eaten since yesterday morning when he inhaled a granola bar and had to drink directly from the tap to keep from choking.
So, when he got said pink-papered eviction notice, Vash knew he couldn’t be picky about his employment anymore. When he initially went job searching, all his hope had spiraled down the drain with every page he sifted through. References, high school diploma, good standing, experience… Every single one listed something that Vash simply didn’t have. By the time he got to the fifteenth page, all the listings had been for such specific types of people that it was no wonder they weren’t getting snapped up immediately. Contractors and freelancers and people with connections that Vash could never even hope to have. All except for one:

It was amazing after all this time that Vash was still extending the olive branch, though this was the most hesitant form of invitation he had received yet.
He felt his lip curl, because Vash always used these tactics, always went below the belt and preyed on his never-ending desire to see his twin. It wasn’t fair, because Nai had now come to accept that the love they felt for each other would never be equal. He didn’t have the open heart that his twin did. He only wanted Vash, and it had been that powerful, gripping love that had driven the wedge between them.
Tonight is the moment he's been waiting for. The final nail in the coffin.
Yes. Vash would find some way to reject him again, and in that rejection, Nai would finally die.

Quietly pulling Vash to him behind the main office table, Knives held his brother loosely by the hips. “You’ll have to recount all this to me when you wake up.”
His brother frowned with an uneasy smile. “...what?”
“You’re dreaming, dearest,” Knives persuaded, voice saccharine sweet.
“No, I’m not…!” Vash let out an anxious laugh, then his face morphed into worry when Knives kept on smiling at him. “Nai, what's going—”

As for what’s going through that thick skull of his, Wolfwood has his guess, and it’s the echo of what Vash had said a few weeks ago.
‘Yeah! Nico gives real good head. You should try it sometime!’
Or later, when Wolfwood had been buried between his thighs, Vash had rolled over prodding his twin, ‘Looks good, right? Give him a chance, Nai.’
OR, Knives and Wolfwood let the intrusive thoughts win.
