Summary
Intern had one goal, finish their Internship and be hired as official staff. They are fully aware the place is a death trap, but the hospital provided a pre-paid apartment, a decent meal plan, and a future.
Intern didn't care about the future just current survival.
If they survived, good, if they died so be it.
They'd been doing fine so far, now if only the others kept in tune with their script.
Or, Intern is a sociopath, and quite a skilled one, but sometimes it's not enough.

I guess life really isn't fair- because now Cosmo is stuck with Prince Sprout, a proud member of the anti-omega political party, an absolute jerk of a man, and... I guess that doesn't matter when his family is... Fine...

"Depollute me, gentle angel,"
Cosmo peered at Sprout with his one good eye, a look of not anger, but something so close to fear on his face. He cowered behind the counter as far as he could. It looked like what you imagine a feral animal trying to appear smaller in front of a predator to look like. Except the so-called predator was Sprout, who looked equally -- if not more -- frightened. "..Cosmo?" Sprout took a tentative step forward. Not a single time did either of the pair blink, as if afraid the other would disappear once out of sight even for a second.
"and I'll feel the sickness lessen less."
TLDR; Cosmo is twisted but retained sanity. Sprout finds him and helps rehabilitate him.

Summary
Dandy got a new bright idea, teaching the inexperienced toons how to survive longer runs without Main toons! Of course, he needed the help of an awesome healer for emergencies and knowing that Sprout is a Main... Dandy decided to ask Cosmo instead!
He obviously accepted, what a kind toon!
So now, Sprout is on his own for two days and two nights. He'll be fine, he can deal with it. He's a Main after all!
Mains are strong, they don't crack under pressure or the simple absence of their best friend. They don't burden toons with their woes and carry everything by themselves! And they absolutely do NOT sleep with stuffed animals.
As they should.
Or: Sprout is struggling to cope with Cosmo's absence, the fanfic.
