“I ain’t got a special someone right now,” Dean assures her kindly, leaning back in his chair, tapping the table that separates him from Doris and her husband, “and I don’t mind working.”
“Well, of course you do!” Doris exclaims.
“I work most holidays, I really don’t mind –”
“No, you’ve got a special someone,” Doris corrects, eyeing him critically, smirking in a way that crinkles all of her many wrinkles, “a woman like me can tell.”

Summary
It almost feels like, if Dean went to sleep right now, he could get it back. Like he would slip right back into the dream, and it would pick up where it left off. But Dean doesn’t even want to, now that he can see that world for what it is: a cheap hallucination, poking at all his weaknesses, going right for the soft underbelly Dean never shows.
All that work. All that work Dean had done over the past few weeks, trying to numb himself and act like he’d forgotten about it. And now it feels like the Empty took Cas all of five minutes ago, all over again.

A post-15x19, forty one year-old Dean gets a taste of what twenty seven year-old Dean went through in 2x20, “What Is and What Should Never Be,” and sees how the shape of his own happiness has changed. It’s with a hesitant grip that Castiel raises his bow again. He knows rather obviously that he’s fully dressed, but he feels naked when he starts playing for the second time. Expression of any type is terrifying in that way, he supposes. Look at me, it says, and see what I am telling you without telling you. The lonely melody creeps from his fingers again.
He finds himself rocking back and forth on the balls of his feet with each broad movement. As though he’s trying to play the violin with his whole body instead of just his hands. It's a strange feeling. I won’t cry, but maybe you could cry for me.
“Cas,” Dean interrupts abruptly between the time that Castiel picks his bow up to end one note and to start the next. “Are you okay?”

Dean blinks and swallows the food in his mouth, staring at Cas curiously. "I would think...you were either on drugs or going insane, why?"
"I'm not inebriated, nor mentally unstable."
"I—I…" Dean takes in a deep breath, then blows it out heavily. "Cas, you're not fucking with me, are you?"
"I'm not," Cas tells him.
"Okay," he says, "fuck it, let's get hitched."
"Thank you," Cas murmurs sincerely, then picks his fork back up and goes right back to eating.

It’s the expression Cas feared, he realizes, all the times he thought about saying it. Revulsion. It makes him feel sick in the way that goes beyond physical, here where there is nothing physical left.
The moment before it happened had been so sweet it covered up all the hurt. For years, Cas had been holding back those words, biting down on his tongue to keep from saying them. And now he had said it, and he knew that it was good, knew that it was worth it. But on the other side there is only this.
