Jan used the blood as a punishment. It would always come back up, laced with water or rabies, and it was a terrible reminder of their monstrosity. Days began to blur, between the starvation and the exercise and the fucked up sleep schedule, and the only thing keeping him grounded was the nightly fang pulling. The pain was a stark reminder of his vampirism, that he had to work harder to become human, because Nandor the Relentless just wanted to be normal again.

Guillermo figures why not, he might as well since he can't reach Nandor. However, things turn out a bit differently than either of them expected.
Nadja and Guillermo need to be girlboss besties and Nandor and Laszlo need to suck it up and talk about their feelings. Everything takes place after the S3 finale.

“You abandoned me.”
It’s not the first thing he wants to say. He says it anyway. A gag reflex; an instinct that runs much deeper than his cursed blood. He could’ve dismissed Guillermo, could have demonstrated his own sovereignty — I flourished without you, human; I took care of myself, all by myself — but his urgency shocks him out of his aloof facade.
He can’t look at Guillermo. He can’t look away, either. The stubbled cheeks are new, joined by tumbling black curls that frame his soft, sloping features. Exhaustion weighing on his shoulders, bone-deep. Hickory eyes older, and heavier.
“Was that not enough? What more could I have given you? What would have made you stay, that I had not already offered?”

“How bad is it?”
To be honest, Guillermo hadn’t expected Derek’s bite to leave a noticeable scar. Or... more noticeable than the typical vampire’s scar, at least. But then, he hadn’t been expecting Derek to come at him when his back was turned... or to take an entire chunk out of his neck when he bit down... or to hit a motherfucking artery while he was at it. And when he thinks about it like that, maybe he shouldn’t be so surprised.
While it was true that most of the damage had healed when he’d begun his vampiric transformation, because it’d taken so long for him to consume human blood, some remained—namely, a nasty keloid scar that Nandor can’t—won’t—stop touching. Not that Guillermo minds Nandor touching him. He just... wishes that the circumstances were different, is all.

Summary
Guillermo is still in the process of recovering from his recent ordeal. In an effort to pull himself together, Guillermo decides to spend a little bit of time dusting and tidying up Nandor's Crypt. While in the process of doing so, Guillermo comes across some of Nandor's "pryvit" papers. Guillermo takes a look and finds that Nandor has apparently given a lot of thought as to whether or not Guillermo is "OK." And also what his final two wishes would be after all the fallout from the duplicate Freddie debacle.
When Nandor confronts Guillermo for snooping, Guillermo refuses to apologize and demands to know how Nandor ended up using his final two wishes.
